This Medical Consent form explains the telehealth services offered by Zera Health and what you can expect from your care. Please read this carefully before proceeding.
Nature of Telehealth Services
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a licensed provider and a patient who is not in the same physical location. The medical information you provide may be used for diagnosis, therapy, follow-up, and/or patient education.
Risks of Telehealth
As with any medical treatment, there are potential risks associated with telehealth, including but not limited to: information transmitted may not be sufficient to allow for appropriate medical decision making; delays in evaluation and treatment; security protocols could fail; a lack of access to complete medical records may result in adverse drug interactions or allergic reactions.
Benefits of Telehealth
Telehealth may improve your access to care, reduce travel time and costs, provide care from the comfort of your home, and allow for more frequent check-ins with your provider.
Your Rights
You have the right to refuse or withdraw consent at any time. Refusal will not affect your ability to receive future care. You may request a copy of this consent form at any time.
Provider Relationship
By consenting to treatment, you acknowledge that an independent licensed medical provider will review your information and may prescribe medication if clinically appropriate. Zera Health is a technology platform and is not the prescribing entity.
Emergency Care
Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
