Program Name

Zera Health SMS Program

You will receive text messages from Zera Health including: assessment reminders, enrollment status updates, provider review notifications, prescription shipping updates, and appointment or follow-up reminders related to your telehealth care.

Opt-Out Instructions

You can cancel the SMS service at any time. Simply text STOP to any message you receive from us. Upon sending "STOP," we will confirm your unsubscribe status via SMS. Following this confirmation, you will no longer receive SMS messages from us.

Rejoining

To rejoin our SMS program after opting out, sign up again through our website at zerahealth.org/intakeform.html and provide your phone number with consent.

Carrier Liability

Carriers are not liable for delayed or undelivered messages. Message delivery depends on your mobile carrier and network availability.

Message & Data Rates

Message and data rates may apply for messages sent to you from us and to us from you. Message frequency varies based on your interactions with Zera Health. For questions about your text plan or data plan, contact your wireless provider.

Privacy

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except for aggregators and providers of the text message services. Age Restriction

You must be 18 years of age or older to opt in to SMS messages from Zera Health. By providing your phone number and consenting, you confirm you are at least 18 years old.
