By agreeing to use Zera Health's telehealth services, you understand and agree to the following:
Telehealth involves the use of electronic communications to enable authorized healthcare providers at different locations to share individual patient medical information for the purpose of improving patient care. By proceeding, you consent to receive evaluation, treatment, and follow-up care via telehealth from Zera Health's authorized providers.
Authorized Providers may include licensed physicians, nurse practitioners, physician assistants, registered nurses, and other credentialed healthcare professionals who are part of your clinical care team.
Telehealth services may require the transmission of health information via internet or telecommunications device, which may include:
The laws that protect the privacy and confidentiality of health information — including HIPAA — apply to telehealth. Health information obtained during your telehealth session will only be disclosed with your consent or as permitted by applicable law for treatment, payment, or healthcare operations.
By using our services, you expressly consent to Zera Health obtaining, using, storing, and sharing your protected health information with necessary third parties (including licensed pharmacies and healthcare providers) as required to deliver your care, as described in our Privacy Policy.
You understand that you must check the Zera Health patient portal for messages, as this is how your authorized provider will communicate important health information to you. If you do not check the portal regularly, your care may be delayed.
For non-urgent questions, you may message your provider through the portal. Providers may not review and respond until the next business day, depending on when the message is sent.
Telehealth sessions may not always be possible. Disruptions of signals or problems with internet infrastructure may cause issues that prevent effective interaction between you and your provider. Zera Health is not responsible for loss of data or information due to technical failures.
The health information you provide at the time of your telehealth visit may be the primary source of information used by your provider during evaluation. Your provider may not have access to your full prior medical records.
You have the right to withhold or withdraw consent to the use of telehealth services at any time without affecting your access to future care. To withdraw consent, contact us at privacy@zerahealth.org or call (561) 277-0156.
You will be informed of the benefits, risks, and alternatives applicable to your treatment during your telehealth visit.
This notice is governed by the laws of the State of Florida. Any disputes arising from the use of Zera Health's telehealth services shall be resolved by binding arbitration in Miami, Florida, in accordance with our Terms of Use.
