Our Responsibilities

We are required by law to maintain the privacy of your health information, provide you with this notice of our legal duties and privacy practices, and notify you following a breach of your unsecured protected health information.

How We May Use and Share Your Health Information

Treatment: We may use and share your health information to provide you with medical treatment. Payment: We may use and share your health information to bill for your services. Healthcare Operations: We may use and share your health information for quality improvement, training, and administrative purposes.

Special Protections

Certain health information receives special protections under federal and state law, including mental health records, substance abuse records, and HIV/AIDS information.

What Information We Collect

We collect the following information: name, email address, phone number, date of birth, health information (height, weight, medical history, symptoms), shipping address, payment information, device data (IP address, browser type), and usage data (pages visited, time on site).

How We Use Your Data

We use your information to: provide telehealth services and connect you with licensed providers; process payments and fulfill medication orders; send SMS and email communications related to your care (with your consent); improve our website and services; and comply with legal requirements.

No Sharing of SMS Opt-In Data

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except for aggregators and providers of the text message services used to deliver our messages. All other categories exclude text messaging originator opt-in data and consent.

Cookies & Tracking

We use cookies, Meta Pixel, Google Analytics, and Google Ads tags to analyze website traffic, measure ad performance, and improve our services. You can disable cookies in your browser settings. Our cookie and tracking technologies do not share SMS consent data with any advertising platform.

Data Security Practices

We use industry-standard SSL/TLS encryption to protect your personal and health information in transit and at rest. Payment card information is processed by Stripe and is never stored on our servers. We maintain HIPAA-compliant administrative, physical, and technical safeguards.

Your Rights

You have the right to: access the personal information we hold about you; request correction of inaccurate information; request deletion of your information (subject to legal retention requirements); opt out of SMS messages at any time by replying STOP; opt out of marketing emails by clicking unsubscribe; and file a complaint with the U.S. Department of Health and Human Services.

Age Restriction

Our services, including SMS messaging, are not directed to individuals under 18 years of age. By providing your phone number and consenting to receive SMS messages, you confirm that you are 18 years of age or older.

We may change the terms of this notice at any time. The new notice will be effective for all health information we maintain. We will post the updated notice on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at privacy@zerahealth.org or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
